How much is an exam?
The cost of a DOT exam is $115.
How long does the exam take?
On average, our DOT appointments take about 35-40 minutes. If you come prepared, we’ll have you in and out & back on the road!
Can I go to any doctor near me?
No, only Certified Medical Examiners listed on the Federal Motor Carrier Safety Administration’s (FMCSA) National Registry can perform DOT physicals. All POHS providers are certified to perform DOT Exams.
What should I bring?
If you have any ongoing medical conditions (such as diabetes, high blood pressure, or sleep apnea), bring recent medical records and a list of current medications. If you use corrective lenses, hearing aids, or other assistive devices, be sure to bring those with you as well.

A Note About Medication
If you take certain medications, you may need to have a medication evaluation form completed by your prescribing provider before your DOT physical.
Common Medications That Require a Medication Form
If you are prescribed hypertension medications (e.g. Metoprolol, Alpha Blockers) for disorders unrelated to hypertension, a medication form is needed.
Blood thinners (e.g., Warfarin, Eliquis, Xarelto)
Medications for heart conditions (e.g., Amiodarone, Digoxin)
Certain psychiatric medications (e.g., SSRIs, mood stabilizers)
If you are taking any medications that list “drowsiness” or “dizziness” side effects on the label, a medication form would be helpful.
Seizure medications (e.g., Keppra, Dilantin). If you are being prescribed seizure medication for a non-seizure condition, a medication form is needed.
If you are taking seizure medication for seizures , it is a disqualifying condition and disqualifying medication use.
Sleep disorder medications (e.g., Trazadone, Amitriptyline)
Narcotics or opioid pain medications
Other medications that might require review: Methotrexate, Steroids, Suboxone, Immunosuppressants

Our DOT Providers
DOT physicals must be performed by FMCSA-certified medical professionals, including doctors, physician assistants, nurse practitioners, and chiropractors. Certification ensures they meet the training and standards required to evaluate commercial drivers' health and safety. Always verify your examiner is FMCSA certified. All POHS providers are certified to perform DOT Exams.
