Do I need to bring an ID?
Yes. Only 1 form of Government-Issed ID is needed.
Can I bring family or friends to my appointment?
At this time, only patients with appointments are allowed into the clinic. If you need extra assistance, we can make exceptions.
Children are not allowed at appointments - please arrange for childcare prior to your appointment.
Do you accept insurance?
We do not accept private insurance. Occupational Health services should either be covered through an employer’s Worker’s Compensation insurance. Other occupational health services are either billed directly to the employer or will be due at the time of service.
Do I need an appointment for drug screens/fit testing/audiograms/vaccines?
These services are all completed on a walk-in basis; you do not need an appointment. Our walk-in hours are Monday through Friday, from 8am to 4:30pm.

What should I bring to my Respirator Fit Test?
Please bring the respirator your employer would like you to be fit-tested with.
Make sure you are clean-shaven if applicable; facial hair must fit within the seal of the respirator for accurate test results.
How long does it take to complete a DOT Physical Exam?
This exam usually takes about 45 minutes, from start to finish.
If you give us your email address when you make your appointment, we can send you all the paperwork ahead of time to complete before you come to the clinic.
Do you complete drug screens for personal reasons?
We can complete drug screens if you are required to furnish results to a Judge, Attorney, or Guardian Ad Litem.
We do not complete drug screens and send results to the patient.
